Hi,
I am just wondering if you have a custom theme?
The only reason I can think of which would cause such a result is the theme running though “the loop” in the header for some reason or other. I will however try and recreate this.

I’ve just figured out what the problem is. I when I updated the mysql table structure I forgot to change the install sql. So the SQL tables are incorrect. Please wait and I’ll quickly change the tables and reupload.

That is not a helpful bug report. Is it blowing up random passer-bys? Is it deleting all your posts?
I’m also having an issue, downloaded the plugin today and cannot add questions.
Plugin install is fine, add new quiz, add sections, once the sections are saved I have no option other options. The quiz name does not appear, so cannot add questions. It just takes me back to the start, add new quiz/survey page.
Any help would be great
I’ve noticed a bunch of bugs today since the update was quite a major one with nearly everything being changed in some way or another. So there has been 3 releases today, if you’ve tried on anything less than 1.3.2 then please update and try again. If you are on that version or it keeps continuing to fail then please provide more data such as what the quiz name is what settings you choose etc.
This has been implemented in version 1.3.2

The bug in the contact form will be fixed along with 10 other bugs in the next release which is scheduled for Friday.
The User Interface is undergoing a major overhaul in next release and that page has been changed and noone longer looks like that.
If I have the time I will try and add it in the next release, however I can’t guarantee it.

There is a plan to be able to add this feature in the future. I’m just not sure when it will be implemented. I can’t think of a quick hack that will achieve this either. I will try and have this feature in the next release but not sure if there will be time.
Plugin doesn’t have a widget. Why would you expect it too?

1st 2 errors are fixed in the new release version 1.2.
The session errors are host related and you would need to get them to fix their php install.

Have you tried enabling WP_DEBUG to see if it outputs some sort of error on this page which could help me solve the issue?

This was meant to be removed earlier but I didn’t know and it has been removed in the next release.
To create a survey you need to goto the survey section and click add new survey.

I think it may not be compatable with PHP 4. It hasn’t be tested on PHP 4 at all. I would advise you to upgrade your PHP install as PHP 4 stopped being maintained 3 years ago. If you’re on shared hosting then I would advise you to move hosters, I would suggest http://www.catn.com
