Figured it out. WP Supercache has a conflict with the Plugin. I had to deactivate Supercache, re-install Author Spotlight and then re-activate Supercache. Strange!

Apologies. The site is at http://chubbyhubby.net. The search widget is at the top of the sidebar on all internal pages and posts.

Don’t understand this. The guy who build the theme said the theme uses the WP default search widget.
