I’ve got the same issue but I did wrote the ID correctly, as you can see here http://www.patch-bcn.com/interest-form/
I was working until I reactivated qtranslate, so I guess that it’s linked, anyone knows what could be the problem?

I found it by editing the style.css header h1 with “display: none”
edit: i changed the url of the first link, it’s like this now:
