We tested the plugin by creating wordpress install in it own directory… but the widget is working on our end. I also have Debug mode on and I dont see any errors.
The wordpress blog is on localhost while the wordpress core files are in localhost/wptest

Can you switch the debug mode and tell if you get any File not found errors…
Also which version of wordpress are you using?
