It probably happened because of conflict with a plugin. Please deactivate all the plugins except for this one to see if the issue persists. If the problem goes away, re-activate and test each plugin to see if you can determine which plugin the problem originates from.Hi,

Are you using dList theme? We used dList theme in this demo site.
