The issue is occurring on https://rti-inc.com, not consistently but it occurs when refreshing the website on Safari and occasionally Chrome.

See the Undefined tab showing up at the bottom of the below screenshot.

Just wanted to pipe in to say we are also seeing this issue with Gravity Forms and WordFence.

I checked the wordfence/css/wf-adminbar.<build-number>.css file and the CSS you posted is in there, so it must be an issue of being overridden.
