Found it. Using SecServerSignature hoses up the web server detection. Added (apache) to end of custom signature which got everything working!
The latest update seems to have fixed it. Not sure how or why!
Scratch that… I think this was related to a geo-location plugin for comments. Something strange about the response was silently killing PHP through a CURL error – at least that is my best guess.
