1. Screenshot of the form and workflow settings
2. debug.log after the clicking update button in the table in question ( can be done using this plugin – https://wordpress.org/plugins/debug/ )
3. console.log after clicking the update button in the table in question  ( https://help.mypurecloud.com/articles/gather-chrome-console-log/ )
