[WP125] Strange behavior

I’ve added WP125 ads to my blog (http://www.tallycast.com). I’ve set it for 2 col, 4 ads and currently have three active ads in the list. One of the three appears but the other three slots show ‘advertise here’. What am I doing wrong?
