Have you tried testing for a plugin conflict? To do so, you’d deactivate all of your plugins except for Exchange and then see if it works properly. If it does, reactivate your other plugins one at a time, testing each time to see which plugin is creating the conflict.
You can also test for a theme conflict by temporarily activating one of the WordPress default themes like Twenty Seventeen.
Give that a try and let us know how it goes.

Typical – every update something goes wrong with the iThemes Exchange plugin. It can’t handle other plugins, or most themes or a change on the server or Mondays.
I turned off the other plugins and I uploaded an image. So the problem is iThemes Exchange. All the other plugins work fine. Please fix iThemes Exchange so it works with the other plugins.

Unfortunately, there are thousands of plugins available for WordPress. We cannot always guarantee that Exchange will work with every available plugin.
If Exchange works with all other plugins disabled, then the issue is the other plugin, not Exchange. Did you enable them one at a time testing each time as I recommended to see which plugin is creating the issue? Even if the issue is specific to Exchange, if you don’t let us know which plugin it’s conflicting with, there is nothing we can do about it. So please test to see which plugin is creating the conflict.
Are you able to add images other places, like posts or pages, when you have all your plugins enabled?
