    I will re-test a month from now as apparently you’re still maintaining your plugin.
    Here’s my advice:
    1. Hire someone for interface and style, the current gives a very bad first impression
    2. don’t add the header “Contact us”, admins usually add the form to a dedicated page or widget and they will add the title.
    3. submit does not work for me… honestly, it doesn’t have to be ajax so remove that too.
