user database plug-in recommendation?

I am setting up a website for a childrens’ choir. I would like a member database plug-in with customizable fields – address, phone, emergency contact, etc. Ideally I would like for new choir member families to be able to fill out these fields. Should be able to produce mailing lists, and email groups.
