upload image box disappeared

Hi,

I have a odd problem.

The problem is when I upload a image to a post a second time.
The first time everything goes well.

Look at the image here, everything ok:

When the first image is uploaded and I press the upload link under the box the second time I get this:

As you can see the upload image box that you normally see is replaced with the top menu bar.

How can I fix this? Any ideas?
