Try to reactivate the plugin first in the plugins pages.
It’s still blank. There’s no form builder showing at all.
Now that is really weird if you don’t see the form builder. What version of WordPress are you using?
I need to see the error otherwise I can’t resolve this issues. I’ve tested this plugin on several servers and I don’t see the same errors that you had. I can fix this if you will allow me to have access only for the GuiForm pages.
How do I let you have access? It’s just a gray space showing instead of the form builder.
I tried on both Firefox and Chrome and it’s still a blank page. I would attach an image but there’s no way to on here.
