Unexpected Behavior

Hello,

I installed and configured the plugin however when I enable it on one of my forms. map fields and associate it to a contactus.com field I get the following error:

The Email and Name cannot have the same CF7 values. Please change one and try again.

I also see the following in my error log:
mod_fcgid: stderr: WordPress database error Table

‘nhsc_wp.wp_evr_options’ doesn’t exist for query SELECT * FROM wp_evr_options made by require_once(‘wp-load.php’), require_once(‘wp-config.php’), require_once(‘wp-settings.php’), do_action(‘init’), call_user_func_array, evr_init, evr_createCompanyArray, referer: http://nhshoppingcenter.com/wp-admin/admin.php?page=wpcf7&message=saved&post=12552&cf7cloud_errors=The+Email+and+Name+cannot+have+the+same+CF7++values.+Please+change+one+and+try+again.

Any thoughts on whats causing this error?
