    • The URL to the site where your plugin form lives so I can test it out.
    • A link to the hosted signup form for the list that is connected in your plugin.
    • What version of the plugin are you using? If you find an update is required, go ahead and update and test again.
    • Have you made any changes to the plugin code?
    • What theme are you using?
