Survey Monkey PopUp Embed

I am currently trying to embed multiple Survey Monkey auto-generated codes for pop-up invitations to take different surveys. Ideally, I would like them on separate pages and not to pop-up on the home page or other posts/pages. Is there a way to have them on their respective pages, for example: Thrift Store Customer Satisfaction Survey (Survey Home: CareNet Thrift Store); Volunteer Survey (Survey Home: Volunteers) (visit link below to see what I mean)
