I go through your site and it seems like there is some error in your form. Some fields are not visible properly and the form submission is not working as well, When I try to submit the form it says “The email is required” but there is no email field appears for me.
can you please check your form fields?

thanks for the replay. Yes there are 3 hidden fields in the form which is auto field because this form is only for some already login users and form submission is working properly as i have tested it.
please help with solution.
