Submit Info Plugin

Trying to get a few suggestions. I been searching different plugins for forms but need one that can do attachments.

Iam trying to have a submit info box basically with attachments.
Any help on one to try is great.
This is what it needs to do:
Needs to have a few fields that can be customized, have a large area for a user to insert text and also Needs an option for attachments to be sent.
