Square mis-formatting credit cards

I need urgent help. The credit card field on the square plugin is allowing for bad formatting of card numbers. See the following, I am able to enter cards successfully sometimes in the format XXXX XXXX XXXX XXXX, other times the card is misformatted and restricted like this:

https://i.snipboard.io/W9NHVE.jpgThere is no rhyme or reason that I can tell. Just entering random numbers a bunch of times causes the same issues over and over.
There is an error regarding this plugin which shows in the console. I checked and this error does show even without my custom theme installed. The file is in this place correctly, it just returns a 404 when visited:

I’ve tried disabling ALL plugins on the site + switching to the default theme and nothing helps, the error persists.
