please try to disable the currently activated plugins and see if the error persists.
If that doesn’t solve the problem you can also activate the default theme to narrow down the bug.
Your menu icons are also missing so I assume there’s a conflict with a script.

It’s a fresh install so there’s no installed plugins on it, and I have double-checked I found 2 inactive plugins there.
There are 3 themes available, I have tried them all nothing seems fix the problem.
“Menu icons” do you mean WP Admin top navigation bar ?

Yep, I tried all default themes that comes with wordpress (Twenty twelve, Twenty fourteen, twenty thirteen).. for each one I have cleared my browser cache before I’ve activated it, BUT it doesn’t fixed the issue.
I also tried both version 3.6 and 3.7.1 and the problem still happening.

This issue happened when I decided to update the wordpress from an old version to 3.8.1
And the problem still occurs even when I reinstalled the old version.

Nope, sorry I meant nothing changed after I’ve enabled the debug log. the page looks exactly the same as it was.
Just to let you know, here’s what I have done to enable the debug log: I have changed this code “define(‘WP_DEBUG’, false)” to TRUE in the wp-config.php file.
Is that correct ?
