SKU QUERYSTING VARIABLE

In Theme Options>Product Page>HTML AFTER ADD TO CART BUTTON (GLOBAL) I’ve added button shortcode which links to ninja forms contact us form. I would like a text area pre-populated with either the SKU or product title or product URL using the querystring variable option in ninja forms.

Can anyone help with what I need to do in order to pass these variables to the ninja form? I have minimal code experience but am learning and can follow instructions well.
