Sample Contact Form on Demo

Hi there,

does anyone know which plugin was used to display the contact form used on the Baskerville demo style-guide page? In my installation there was no styleguide page on a sample page that had none of the style formats as found here: http://andersnoren.se/themes/baskerville/style-guide/
