Revision notes next to list

I am looking for an existing plugin that may accomplish what we are looking for. I will do my best to explain the requirement.

After a page has been created WP gives us a nice list of revisions for those with the permission to do so. What it lacks are specific notes related to each revision…AND that they show up next to the revision list.Without that additional data it is too hard to comb through revisions with alot of editors on a project.

Any help pointing us to the right plugin would be appreciated.
