To add registration to your account page,
Go to Dashboard-> Woocommerce-> Settings-> Account Tab
under registration option, select enable registration on the “My account page” and save.

Thanks for the reply,one more thing i want to ask that is I want that on particular product page,I want to display extra fields. I am sending the screen shot of it do check the url http://prnt.sc/b5woou. Do help a bit confused how to do it.
