When a user fills in and submits the signup form, the normal behavior is to take them to the thank you page.
I don’t understand what it is you want to happen…a “list” is not something that a user can fill in. What is it you want the user to do?

ok. i am not completely clear sorry;)
1. made the sign up form;
2. placed the sign up short code on the specific page;
3. the button appears on the page;
4. by clicking the button, i am not directed to the sign up form but directly to the Thank you page. Which means nobody can sign up because the form is not showing online in the frontend only in my back end with the plugin.
What i want:
We need volunteers to fill in a application for a Spay and Neuter campaign for dogs in Aruba. Therefore they need to fill in a questionnaire. I thought this would be a great replacement to do it online plus all is in a database. This is possible right?
I hope you can help me, i went through all docs, they all state things about the sign up form itself which was very helpful however i couldn’t see more explanations about the actual ‘ how to place the form online?’

OK you need to tell it which fields you want in the sign up form. On the “manage database fields” page in the Participants Database admin menu, each field has a “signup” checkbox. Check the ones you want in your signup form.
Make sure it is the correct checkbox, it looks like you don’t have any fields selected to show in the signup form.
it are the check boxes at the end of each line correct?

OK, then check your field groups, the group with your signup field needs to be checked “public”
ok one more obstacle, the private link in the confirmation error gives a 404 page not found.
make sure the “participant record page” setting (under the “record form” tab) is correct.
