Processimng HTML Form HELP!

I ahve built a form with html and javascript over here. But i don’t know how to submit the form and send it to me, and known of the online ones worked out for me, i’d like to know how the forms at https://www.canadavisa.com/assess/canada-immigration-assessment-form.htm is submitted, ithink that would work for me, inoe its probably a very silly question, but i’m not too good with forms,, thanks a lot in advance
