problem with layout

If you remove the space between the different tags in the form editor this will help you with your breaks and spacing. To add the Field name next to the input you would simply add the text you want right next to the tag

Be sure that the tag and the text are on the same line, this will keep them together. Do you have a reference link that we could look at if that doesn’t fix if for you.

looks like you have a style for the ‘span’ elements that hold the inputs set to width 100% this is keeping them and the text on separate lines
