Probably a simple answer!

I have built a very long and frankly ugly looking form with lots of fields we need to capture data on.

However, I also have a requirement to have the answers in the form answerable by two applicants at the same time, so the first question is your name, I want the name of applicant 1 & also applicant 2.

Following on from there, we need the dates of birth for both and also the answers to the yes/no questions etc etc.

I was thinking of having two forms altogether, a single appicant form & a joint applicant form, linking them to an initial button press on the web page before directing them to the relevant form. However I cannot for the life of mne figure out how to have the questions to have multiple answer inputs! Any ideas?

The template I am building this off is a spreadsheet, if I could get this online and colelct the answers that would probably be ideal.
