[Plugin: WP Responder] Subscription Form Problem

I really like this plugin. It is perfect for what I need. But this thing has issues. The subscription form creator will not save the form so that it can be used. The autoresponder will not save the messages. I have been driving myself crazy googling the entire internet for answers and I can’t figure out why it won’t work. I was able to import all of my list and my newsletter is set up.

I am designing this site on a localhost. I am running xampplite with phpmyadmin 3.2.4 and php version 5.3.1. The plugin is not putting out any errors, it just will not save the forms I need to create in order to proceed. When I try to create a form and I choose custom fields it creates html with only the name and email fields no matter what fields I choose. I have emailed the developer but I have not heard from him yet. Any help at all would be appreciated.
