Plugin Request

Hello, Is there any conditional form plugin out there that can resolve the following processes:

An employee completes a form with conditional logic

Form is sent to line manager’s email

Form is endorsed and commented by line manager

Form is routed to head of department

Form is endorsed and commented by head of department

Form is routed to Dean

Forms is approved by Dean

Employee gets approval email

In all these processes employee receives email notification to be updated on progress.

A final draft email contains form elements of all processes involved.

Is there any plugin or system out there that can handle this?
