[Plugin: Contact Form 7] CF7 and Simplemap

Is there a way to have an embedded contact form work with simplemap?

Simplemap generates a distributor list based on distance from your location, so it’s dynamic, and I’d like to have a “click here” contact form to reach an individual distributor…if that makes sense.
