Plug-in recommendations

Hello:

So I’m trying to set up a change request system, but I’m getting a little over my head in what plug-ins to use. I’ll lay out the basic flow of what I want to happen.

1. Requester visits site and completes a form
2. Form info writes to database and is emailed to supervisor
3. Supervisor receives email and can approve/deny/modify the form
4. Upon approval, email notification is sent to requester as well as multiple directors
5. Directors can approve/deny/modify the form
6. Once 3 directors have approved the form, notification is sent back to requester to proceed

I want the database to automatically assign each request a unique ID, that anyone can use to check the status of the request (where its at in the approval process).

I’m using FS Contact Form and have no idea what plugin to use to write the form data to MySQL, and no idea what plugin to use to allow users the ability to track the status of the request. Anyone done anything like this before?
