I am having the same trouble. I have placed the shortcode in the page I want the player displayed. Tried adding:
AddType audio/ogg ogg
AddType audio/ogg oga
AddType video/ogg ogv
AddType video/mp4 m4v
to my .htaccess with no success, and made sure I have the most update version with no luck.
Also, when I try to use the upload buttons on the player manager, they don’t work and I have to go to media and copy and paste the song and image urls. I have tried uninstalling and reinstalling the plugin, and now it is even more buggy. In addition to the upload buttons not working, now songs aren’t even being added to the playlist after clicking Add Song.
I really like the look of this plugin in the screen shots, and like the features it offers, but after two days of trying to get it to display and upload songs, I am at a loss.

Bug fix for the plugin incompatibilities will be released this coming Tuesday. Happy to say I have finally managed to fix this!
Also I checked your code and can see the reason the player isn’t showing is that you added audio file with the extension .m4a instead of .mp3

Ok sounds like some major issue/s going on with yours! Possibly a conflict with another plugin so please may I ask you to try disabling all other plugins and revert to the default twentytwelve theme and see if the plugin behaves better? Please report back for further assistance… Oh and also for future reference if your issue has nothing to do with the original issue being discussed in a thread it is best to post your own new thread. I recently updated to WordPress version 3.5.2 and now my audio player is not showing in my post. The audio is showing in the post but not via the player. it simply says the name of the song and you have to click on it… Can anyone please assist me with this??? The add song button is not available anymore…

Yes you need to add the player to your page or post using shortcode [hmp_player]
