I don’t want to give out a link for the live site, but I will consider making a copy of the site, with a test petition.
In Nov. 2019 we did a test because we noticed that one of the people who tried to use the contact form on the site (Ninja Forms), and had it fail, was using a mobile device and a VPN. We tried with and without a VPN from a computer, from our end, and confirmed that the form failed when we used a VPN. We believe this same failure is occurring with SpeakOut! forms.
Further, we determined that the VPN problem was specific to the WordPress site, because we didn’t see this failure with another website, with form submission (same server, not WordPress).

We figured out the problem: it was because the firewall on the server was blocking unauthorized access to wp-login.php and /wp-admin.
Is the plugin using files under wp-admin?

They are part of the core so indirectly the plugin will be using them.
Curious…is your server using Imunify360? I found some cpanel/WP issues not related to SpeakOut!.
