If Double Opt-in is enabled, then once the user submits our form, MailChimp.com sends the user a verification email with a link. Unless they click the link they are not added to the MailChimp list.
To debug this I would first suggest checking if any errors are listed at “MailChimp for WordPress > Others” menu in your WordPress admin. If there are no errors then try disabling “Double Opt-in” and testing again.
