No forms are displayed

Hallo,

I have a specific problem and I can’t find its solution anywhere. I would be glad if you could help me.

After creating form and putting the shortcode “[contact-form-7 id=”1708″ title=”Kontaktní formulář 1″]” into my post page, it just display titles but no text fields.

I looks just like this:
Your name (vyžadováno)
Your e-mail (vyžadováno)
Subject
Your message
(Submit)

But instead of the submit text there is real submit button. (see the web)

The code for the form is this:

<label> Your name 
    [text your-name] </label>

<label> Your e-mail 
    [email email-145] </label>

<label> Subject
     </label>

<label> Your message

Do you have any ideas why it does not show any text fields? Thank you!
