There are two options: Settings and Extensions. There is no way to upload a file.
How do I do this in the backend or (bettert yet) frontend?

Above the “Settings” is there no “All Downloads” & “Add New” option?

That is strange. Can you please let us know which version of our plugin is installed and also the PHP version on your server?

Currently WordPress 4.9.8 is running Download Monitor 4.1.1 on a PHP 7.1 server.
