Need help finding a 'subscribe to' widget/plugin.

    Hey there, I have searched high and wide for a plugin that gives me the option to:

    1) Add a ‘subsribe to’ box for email subscription
    2)allow reader to follow me on twitter
    3)like me on facebook

    Id like to do this all on the same widget.

    http://www.problogger.net has the exact widget im after (on the right sidebar) but i cannot find the name of it to save my life!
