Need contact form recommendations

    I need to customize a contact form for my business website. In addition to name, address, email and phone, I need to know the person’s company name and website. I also need three message boxes:

    What project do you have in mind?

    When do you need the finished project?

    What is your budget?

    I tried Ninjaforms, but couldn’t figure out how to add the above items. I’m not code-savvy. Any recommendations for an easy, customizable contact form?
