Need a good forum plugin

Hi, trying to build a local review site for tourist visiting my local area and would like a plugin where people can leave reviews. What I am looking at is two things.

First of all I am looking for a forum plugin like this http://www.holidaytruths.co.uk/forum/ or like what trip advisor has, I am also looking for some sort of form that people can fill in like the below

I want the site to be user friendly for my local area where people visiting the local area can let people know what they thought of the accomodation they stayed at and places they visited.
