Multiple Registration forms

Hi. I really need some help 🙂

I need to add multiple registration forms, as i run a clan website wich plays multiple games, we also have multiple clan leaders for each game, wich would like a custom registration form for each game, then for them to read trough the registration form and approve or cancel. Before the user gets acsess to the page.

Is this possible? Thank you. 🙂
