Multiple Forms

2. Major Problems:

1) I have 4 forms on a single page, and only the first upload form appears, the remaining 3 show: “Your browser doesn’t have Flash, Silverlight or HTML5 support.” error, no errors in browser console appear.

2) Files are not uploading.
