Multiple controls Error & Not receiving email

I thought I was an experienced user of Contact Form 7 but I have been stumped for several weeks on this. I am receiving error :! Multiple form controls are in a single label element. I followed instructions on to remove the wrap text and use label element, but I Receive the same error. Additionally, I am not receiving the email once the user completes and submits the form indicating it was sent successfully.
I have attached both my Form and Mail code:
1.Can you tell me why I am receiving these errors?
2.Can you tell me why I am not receiving my email?
Any Help you can provide would be greatly appreciated
