Missing buttons

Hi, When create a New From, I note all buttons are missing, “grid” and “html” but also all field buttons. I’ve an error message ” the editor failed to load. You may recover your form by copying it into a new form editor, do you wish to continue? ” copy the form change nothing. Could you help on it ? Thanks
