I have the same problem.
Mine started after updating to latest wp version or when i updated php5 to latest version. (Did both updates without checking the map function). Before that everything worked fine.
I have tried deactivating the plug in and delete all data, no luck.
I get this from apache/error.log each time i Activate the plugin:

I have optimized the database in an attemt to get rid of the duplicates as the errormessage suggests, lo luck.
I need to fix this problem, this plugin is awsome.

I had the same problem.
It appeared to be that another plugin was conflicting with this and several other plugins. I deactivated that plugin and now it is working perfectly. By far my favourite google map plugin!

Also, who have same problem, please check your theme – does it have wp_footer() function call in footer.php (it should have it), if you don’t know how to check this – just try to activate standard wordpress template: if map will start to show – then problem is in your template.

Resolve them at first – then send me a message here. Or just deactivate those plugins (nimble-portfolio and featured-pages-customizer)
