You’re getting errors related to your api in your javascript console.
It seems that the site is not attached to your current API. You need to go into the settings for the api and be sure that the url allowed is exactly the same as the page that you’re adding the map.

There weren’t any restrictions, I just added the page URL as an HTTP Referrer, will this fix the issue?
I’m not sure of the terminology used. I haven’t created an API for maps in since they started requiring credit card information.
Previously you could leave the area where it asks for URLs blank, but I would just add the URL of the specific page that you’re adding the map.
If you continue to get the api error on your contact page, the you’ll know that there is an issue with the permissions of your API.
