Making a Survey ?

I am searching for a survey, that would generate the results in a nice matrise. It would calculate from the scores and weighted average and display a matrise. It could also return a text with some feedback of the result. Where could I find this?

Secondly I wonder if its possible displaying archived e-mails or text-messages(sms) in wordpress? is there an extension that would easily display these?
