Mail Tags Not Populating

I have followed the instructions to creating my multi-step form and the mail tags don’t seem to be working once the user submits the form. They aren’t populating with anything. I just get:

Message Body:
[text* Your-Company]
[text* Industries]
[text* Titles]
[text* Sizes]
[text* Territory]

(these are the form fields) even though there was data entered into each. The mail tab on my last form/step says at the top:

In the following fields, you can use these mail-tags:
[your-name][your-email][Your-Company]

And my other fields are not included/available.
