Hi,

Are you using a child theme?
If yes, do you have any “custom” template in child theme, such as header.php?
If yes, you will need to update your templates’ code by comparing with the templates in parent theme.

It can’t find the image because the URL isn’t valid. Try changing the link to this:

http://underground-secrets.org/static/uploads/index.png

You may need to change your header background color to make the image visible.
